Caradigm has launched its new population health suite called 'Care Management' to allow healthcare firms to effectively transition to value-based care models and accountable care.
Developed in collaboration with Geisinger Health Plan, Caradigm Care Management helps healthcare firms in managing the care of populations in order to improve outcomes and the patient experience in addition to reducing costs.
Geisinger Health Plan is a population health management firm focused on new case management models as well as incentive-based wellness programs.
Caradigm vice president of Population Health Brian Drozdowicz said the Care Management will allow Accountable Care Organizations (ACOs) and other healthcare firms to coordinate care throughout every member of a patient's medical neighborhood and support network - including primary care providers, hospitals, specialists, family and community resources.
"They need the collaborative framework and analytics to manage a care plan to the optimal clinical and financial outcomes - no matter how rapidly a patient's condition and priorities change," Drozdowicz said.
The new application provides up-to-date clinical, financial and operational data to clinicians within their workflows, which allows care teams to use analytics to better understand patients, coordinate on their care as they move from hospital to clinic to home as well as respond to their changing needs.
With the new Care Management, healthcare firms can achieve the goals of the Institute for Healthcare Improvement's Triple Aim, which include improving the patient experience of care and the health of populations as well as reducing the per capita cost of care.
