Kryptiq, a privately held software company, has announced the release of new version of its population health management solution, CareManager 4.0.
Co-developed with leading healthcare organizations, and curated in collaboration with a 42-member customer advisory group, Kryptiq's CareManager aligns the care team with the decisions made at the point of care.
Unlike the alternatives, CareManager is deeply integrated within the EHR, is informed by real time clinical data, and provides integrated patient engagement.
Color-coded dashboards present a simple and easily recognizable view into patient risk across the population.
Treatment protocols are recommended via CareManager's customer curated guidelines, enabling all providers to practice at the height of their licensure.
CareManager extends the provider's workflow to the patient by generating individualized care plans and patient-friendly, color-coded scorecards.
Providers use the scorecard to engage patients in the encounter room, and the opportunity for personal involvement is enhanced because their scorecards are automatically delivered via their patient portal.
Additionally the population registry enables quick identification and management of patients that are at risk or symptomatic of a chronic illness, but lack a formal diagnosis.
Kryptiq population health management general manager Malcolm Costello noted in the brave new world of value-based payment, healthcare organizations must shift care delivery from seeing 25 patients per provider per day, to managing 2500 patients per provider.
