Truven Health Analytics has upgraded its Health Unify population health management solution with healthcare data warehousing, analytic methodologies and decision-support capabilities to improve patient outcomes and reduce costs for clinical providers and care managers.
Enhanced by the open source Apache Hadoop framework and deployed in partnership with CareEvolution, Truven Health Unify is a cloud-based solution that is now interoperable with all major clinical health information systems and health information exchanges.
The solution allows health systems, accountable care firms, hospitals and health plans, and health information exchanges to effectively manage population risk, provider networks, clinical outcomes, population health management and patient population messaging and engagement.
Key features of the solution include analytics for population health and care management, data expertise, customizable delivery, and payment reform expertise.
